The Diagnostic Value of CD11b Expression on Peripheral Blood Neutrophils for Detection of Spontaneous Bacterial Peritonitis

Background: Spontaneous bacterial peritonitis (SBP) is associated with the highest mortality among end-stage cirrhotic liver disease patients. Neutrophil CD11b expression increases on neutrophil surface within 5 min of exposure to bacteria. Paracentesis remains only accepted method for accurate evaluation patients, many drawbacks; hence, a diagnostic noninvasive marker very high sensitivity and accuracy necessary. Aim study: evaluate as non-invasive biomarker diagnosis SBP, comparing its specificity other traditional methods. Patients Methods: 200 patients who had cirrhosis ascites were recruited Hepatology department inpatient wards National Liver Institute, Menoufia University. They divided into Group I: 100 SBP II: non ascites. All studied subjected full clinical examination, abdominal ultrasound, paracentesis, laboratory investigations including ascetic fluid (AF) examinations. The mean fluorescence intensity (MFI) assessed peripheral blood neutrophils by flowcytometry. Results: There was significant increase in MFI group compared group. At cut off >20 100% can discriminate between cases followed TLC examination at 0.26 (×103) 92%, 96%, then, AF count 0.25 80%, 100%, culture 56% 100%.
